Renewal of Passport:

If your passport got lost due to unavoidable circumstances, you could apply for re-issuance of the passport. But before applying for passport reissuance, you need to report the loss to the local police. And you need to submit the police report along with your application and other documents.

  • Application Letter stating the circumstances/location of the loss.
  • Police Report of Lost Passport.
  • In case of loss due to a fire outbreak, a Fire Service Report is required.
  • An affidavit from a Magistrate or High Court of a lost passport is deposed to by the applicant, with the applicant’s passport photo attached.
  • Acknowledgment Slip and proof of payment.

Note: If a passport was declared lost and reissued, the holder must immediately notify the Nigeria Immigration Service of the recovery and submit the passport for cancellation.

Name Change Due to Marital Status:

Do you have any recent changes in your marital status? And have you changed your name as a result of it? If yes, you must also change your name on your passport. If your passport does not reflect your current name, you may be denied entry into an airport. So, refer to the below sets of documents and apply for a name change in your passport.

Married:

Are you married recently and want to change your name in your passport? Below is the set of documents that needed to change your name in your passport.

  • Application Letter
  • Copy of Declaration of Marriage or Marriage Certificate.
  • Sworn Affidavit of Change of Name deposed to by applicant from a Magistrate or High Court.
  • Copy of Change of Name Newspaper Publication (21 days after publication).
  • Statutory proof of name change (instead of iii and iv if resident outside Nigeria).
  • Passport Booklet and Passport Data Page Copies.
  • Payment Receipt and Acknowledgement Slip.

Divorced:

If you are divorced recently and need to change your name in your passport, submit the below documents.

  • Application Letter.
  • Decree Nisi and Decree Absolute or Divorce Declaration Proof of Maiden Name.
  • Certificate of Marriage with Current Spouse in the case of re-marriage.
  • Applicant’s affidavit of change of name from a Magistrate or High Court.
  • A copy of the newspaper article announcing the name change (21 days after publication).
  • Statutory proof of a name change (if resident outside Nigeria instead of v and vi).
  • Passport Booklet and a copy of the passport’s data page.
  • Payment Receipt & Acknowledgement Slip.

Widowed:

You will need the below documents if there is a change in your marital status.

  • Application Letter.
  • National Population Commission (NPC) Certificate of Death of Spouse.
  • Proof of Maiden Name.
  • Certificate of Marriage with Current Spouse in the case of re-marriage.
  • Applicant’s affidavit of change of name from a Magistrate or High Court.
  • Copy of Change of Name Newspaper Publication (21 days after publication).
  • Statutory proof of name change (in place of v and vi if resident outside Nigeria).
  • Passport Booklet and a copy of the passport’s data page.
  • Payment Receipt and Acknowledgement Slip.

If you have any Correction or Error(s):

If your reissued passport has some errors, you can apply for correction of errors with the below documents.

  • Application letter outlining the reason(s) for the change.
  • If you have more than one [1] alphabet, please attach proof of payment of a non-refundable Administrative Charge of N30,000.00 only.
  • Document evidence from the past to support the request.
  • Applicant’s affidavit of correction of Error from a Magistrate or High Court.
  • Passport Booklet and Data Page Copies.
  • If your passport re-issuance application is accepted, you will be sent an acknowledgment slip and proof of payment for your passport fees.

Other Reasons:

Other reasons you can apply for re-issuance of passport is,

  • Replacement of names
  • Addition of names
  • Deletion of names
  • Rearrangement of names
