E-Passport Renewal Requirements:

Requirements for Philippines passport renewal Dubai
  • Form No.2 – Renewal Regular Passport/Travel document Application (Adult).
  • One OVF 1 (Open Virtualization Format) with Updating of Applicant Photograph & Sign.
  • Online Appointment for passport renewal with QR code.
  • The personal appearance of the applicant is a must.
  • Old Passport original
  • Accomplished E-passport application form & OAV form.
  • Two passport data pages copies
  • Additional supporting documents that may be required as necessary, especially for applicants with incomplete place of birth in old passport, with dual citizenship and with foreign-sounding family names to prove citizenship.
  • All civil registration documents (i.e., marriage contract, birth certificate, death certificate) must be issued by the (PH Statistical Authority – PSA), while documents from the PH court must be apostilled by the (Department of Foreign Affairs – DFA).

Renewal of Brown passport or travel document issued prior to 01 May 1995:

Requirements:

  • Form No.2 – Renewal Regular Passport/Travel document Application (Adult).
  • One OVF 1 (Open Virtualization Format) with Updating of Applicant Photograph & Sign.
  • Book Online Appointment for passport renewal with QR code.
  • The personal appearance of the applicant is a must.
  • Bring original brown passport/travel document & photocopy of passport/travel document pages 1,2,3,4, & last page.
  • Birth certificate (BC) (PSA-issued) to determine the complete middle name.
  • Accomplished E-passport application form & OAV form.
  • Two photocopies of the Travel document Data page.
  • PSA-issued Report of Birth if born abroad.
  • ID Cards.
  • Additional supporting documents may be needed as necessary.

Renewal of Green passports or travel document issued after 01 May 1995 (or) Renewal of Machine Readable Passport issued in the year 2007:

Requirements:

  • Form No.2 – Renewal Regular Passport/Travel document Application (Adult).
  • One OVF (Open Virtualization Format) with Updating of Applicant Photograph & Sign.
  • Book Online Appointment for passport renewal with QR code.
  • The personal appearance of the applicant is a must.
  • Bring original brown passport/travel document & photocopy of passport/travel document pages 1st & last page.
  • Birth certificate (BC) (PSA-issued) to determine the complete middle name. Transcribed birth certificate from the LCR is needed/required when PSA (BC) birth certificate entries are blurred / unreadable.
  • Accomplished E-passport application form & OAV form.
  • Two photocopies of the Travel document Data page.
  • ID Cards.
  • Additional supporting documents may be needed as necessary.

For 17 years old & below (minor) applicants renewing PH travel document/passport:

Requirements:

  • Form No.3 – Regular Passport/Travel document Application for a minor.
  • Confirmed Online Appointment with QR code.
  • Photocopy of valid passport/travel document of either parent (if minor is a legitimate child) / and of the mother.
  • Personal appearance of either parent (minor is a legitimate child) / of the mother (if minor is an illegitimate child).
  • Accomplished E-passport/E-travel document application form.
  • Original & copy of the (PSA-issued) Birth Certificate of a minor. A report of birth duly authenticated by PSA is required if a minor is born abroad.
  •  Marriage Certificate (PSA-issued) of minor’s parents.
  • Additional supporting documents may be required / needed as necessary.

Applicants who availed of Dual Nationality or Election of Philippine Nationality under RA 9225:

Requirements:

  • Form No.1 – New Regular Travel document Application (Adult)Form No.2 – Renewal Regular Travel document Application (Adult).
  • Form No.3 – Regular Travel document Application (Minor).
  • Order of Approval.
  • Identification (ID) Certificate.
  • Report of Birth (for those born abroad).
  • Oath of Allegiance.
  • Additional or supporting documents that may be needed as necessary.

Replacement for Lost Travel document/Passport:

Please note that there will be 15 days clearing duration before processing the application form to replace the lost valid travel document.

Requirements:

  • Form No.1 – New Regular Travel document Application (Adult).
  • Form No.2 – Renewal Regular Travel document Application (Adult).
  • Form No.3 – Regular Travel document Application (Minor).
  • Affidavit report (Passport Loss).
  • The personal appearance of the applicant is a must.

  • Original and two photocopies of Affidavit of Loss executed at the Philippines Consulate General.
  • Original and two photocopies of Police Report issued by Dubai Police with English translation.
  • Additional supporting documentation may be needed as necessary.

Requirements:

  • Form No.1 – New Regular Travel document Application (Adult).
  • Form No.2 – Renewal Regular Travel document Application (Adult).
  • One OVF (Open Virtualization Format) with Updating of Applicant Photograph & Sign.
  • Confirmed Online Appointment with QR code.
  • The personal must visit the application center.
  • Original old travel document/passport.
  • Accomplished E-passport/E- travel document application form and OAV form.
  • Photocopy of Passport/travel document Data page (2 copies).
  • Marriage Contract Issued by PSA.
  • Report of Marriage PSA issued if married abroad.
  • Additional supporting documents may be needed as necessary.

For married women who like to revert to their maiden name:

Requirements:

  • Form No.1 – New Regular Travel document Application (Adult).
  • Form No.2 – Renewal Regular Travel document Application (Adult).
  • One OVF (Open Virtualization Format) with Updating of Applicant Photograph & Sign.
  • Confirmed Online Appointment with QR code.
  • The personal must visit the application center.
  • Original old travel document/passport.
  • Accomplished E-passport/E-travel document application form & OAV form. 
  • PSA-issued Birth certificate (BC) to determine the complete middle name. Transcribed birth certificate (BC) from the Local Civil Registrar is needed when the PSA birth certificate (BC) entries are blurred/unreadable.
  • Two Photocopy of Travel document/Passport Data page.
  • Report of Birth (PSA-Issued) if born abroad.
  • If already widowed: Death Certificate (PSA-Issued) of the deceased husband.
  • If marriage is annulled: (PSA-issued) annotated Marriage Contract & DFA-apostil led Court Order (CO) affecting annulment.
  • Additional documents may be needed as necessary.
  • If divorced: Annotated (PSA-issued) Marriage Contract (MC), CTC of the Divorce Decree duly authenticated by the Filipino Embassy or Consulate where the divorce was obtained/by the concerned diplomatic/consular mission in the Philippines. Original CTC of Philippine Court recognition of foreign decree could also be submitted and the PSA Marriage Contract with annotation of the Divorce Decree.
